Medicare referrals

Please bring the following information with you to your first appointment:

• Letter of referral from your General Practitioner
  
• Any medical reports
  
• List of previous and current medications
  
• Your Medicare card
  
• Your bank savings card (to deposit Medicare rebate)

  WorkCover/Compulsory third party (CTP) insurance referrals

If you are being referred under a WorkCover claim or Compulsory Third Party claim, you will need to provide the following:

• A referral from your doctor
  
• Completed Patient and Insurer Form -

We will request approval from the insurer on your behalf and following receipt of approval, we will contact you for an appointment.
